This agreement outlines the terms and conditions of the VIP Dental Membership Program offered by Richmond Landing Family Dentistry.
The VIP Dental Membership Program is a direct membership program offered exclusively by Richmond Landing Family Dentistry to provide patients with access to preventive dental services and discounts on additional dental treatment. This program is not dental insurance and cannot be used in conjunction with insurance or other discount plans.
Membership is valid for 12 months from the enrollment date. All fees must be paid according to the selected schedule. Membership is non-transferable and applies only to the enrolled patient.
Included services are provided based on clinical necessity as determined by the treating dentist. Unused benefits have no cash value and cannot be exchanged for credit or refund.
Membership fees are non-refundable once preventive services have been provided. If canceled early, services already received will be charged at standard office fees, and the patient is responsible for the difference.
